How do I import my patient list?

Once you are logged into your account, select ‘Import Patients’ from the navigation menu on the left hand side of the page. You can download an example CSV at the top of the page. Please clear the example patient from line 2. Please utilize the following guidelines while filling out the CSV.

The CSV should have the following columns:

First Name, Middle Name, Last Name, Date of Birth, Sex, Phone Number, Phone Type, Email, Address Line 1, Address Line 1, City, Zip Code, State, Country

Mandatory fields are: First Name, Last Name, Email

Please use the follow format for each column:

Date of birth: xx/xx/xxxx

Sex: M, F

Phone Number: 555-555-5555 or 5555555555

Phone Type: Mobile, Work, Home, Other

Email: mail@mail.com

Address Line 1: 123 Main St

Address Line 2: Apt/Suite/Unit


Zip Code: 00000

State: NY (2 letter abbreviation)

Country: US (2 letter abbreviation)

Step 1: Once you have completed the file, save it to your hard drive. Next, select ‘Choose File’ and select the correct file on your hard drive.

Step 2: Select ‘Process File’. If you are missing any mandatory fields, you will be taken to a screen with the missing mandatory fields highlighted. You may either delete the patient using the trashcan on the right hand side, or update the missing data. Note, you may also edit the other fields during this time. Once you have updated and/or deleted the patients in question, select ‘Process File’ to upload your patients.

If you were not missing mandatory fields, the file will successfully upload your patients.
